In order to successfully implement a change within an organization, the change agent must assess the organization's culture and readiness for change. In 750-1,000 words, analyze the culture and level of readiness of the organization for which your evidence-based practice project is proposed. You will use the assessment of the organization's culture and readiness in the Topic 8 assignment, during which you will synthesize the various aspects of your project into a final paper detailing your evidence-based practice project proposal.

Include the following:

  1. Describe the organization's culture and explain to what degree the culture supports change. Consider organizational and leadership structure, mission and values, interprofessional collaboration/team engagement, communication, perception of the organization by employees, etc.
  2. Select an organizational readiness tool and assess the level or readiness for change within your organization. Identify the readiness tool and summarize the survey results. Discuss the degree to which the culture will support and sustain an evidence-based practice change. Consider strengths and weaknesses, potential barriers, stakeholder support, timing of the proposal, and resources. Provide rationale.
  3. Discuss what health care process and systems you would recommend for improving quality, safety, and cost-effectiveness for the organization.
  4. Propose strategies to better facilitate the readiness of the organization.
  5. Identify the stakeholders and team members for the project. Include what their duties will be in the evidence-based practice project proposal.
  6. Explain what information and communication technologies are needed for the implementation and how they will be integrated in the setting by the internal stakeholders. Explain how these will help improve nursing practice and care delivery for individuals and populations for your intervention.

The Northwestern Memorial Hospital is dedicated to offer an excellent care service to their patients and focus on strong communication with their patients and patients' family members. However, the hospital is very attentive to adopting changes in its care services according to the advanced technology and treatment process (Avanecean, Calliste, Contreras, Lim & Fitzpatrick, 2017). The mission of the hospital is to provide a better care service to its patients and to improve the healthcare of the community.
The value of the hospital is explored through the shared commitment and responsibility to a single patient. Teamwork, excellence, integrity and priority of the patient are the core value of the organization (She
ington et al., 2017). However, inter-professional collaboration is good in the organization as the care service of the hospital is going through teamwork where each of care providers is responsible for their duty and share information with their co-staffs. Perception of the hospital towards its staff is motivating and positive.
2. Organizational Readiness Tool
The hospital is attentive to using a Fall Prevention Tool Kit that can be helpful to involve patients and the family members of the patients into the prevention process throughout hospitalization. The survey report resulted in more than 20 patients in the hospital falls so the hospital management is ready to use this tool. The care providers of the hospital enhance changes for the old aged patients between 35-100 years old who are diagnosed with CVA.
The hospital is ready to implement physical therapy and occupational therapy exercise programs daily with Range of Motion Every shift for a zero falls case in the hospital (Cheng et al., 2018). The strength of the proposed tool is explored through zero falls cases and it helps patients get better and faster treatment. Sometimes the tool could fall for measures and could misguide doctors. It is the major weakness of this tool.
However, most of the time stakeholders support the utility of this tool and treatment. Proposed ba
iers can arise through lack of skilled staff. The proposal will be conducted within 3 months. Trained care providers and fall prevention tools will be resources. The statement is rational as it included exercise programs, therapies in regularly that is far preferable than the cu
