Essay topic:
Choose a chronic disease from the list below and critically discuss how you would support self- management of your client/patient living with this chronic illness in the community.
Your discussion should include (but NOT be limited to) the following: Main causes and brief pathophysiology (in simple terms - as you would explain it to your patient); impact of the disease on the individual and family/carers; health promotion; cultural safety; empowerment.
Choose one of the following chronic diseases
a) Cerebrovascular Accident (CVA)/Stroke
b) Chronic Obstructive Pulmonary Disease (COPD)
c) Chronic Kidney Disease
All written assessments must align to scholarly standards.
An introduction, body and conclusion (unless specifically instructed)
Academic standards require:
· APA 6th referencing style for in-text citations and reference list;
· adherence to word count;
· a 12 size font in either Arial, Times New Roman or Calibri; and
· a 1.5 spacing
· File type - Word
All assessments should have a title page that specifies:

· Student Identification number
· Title of assignment
· An accurate word count including in-text citations, but not the end references
· The use of 3rd person (unless specifically instructed)
Evidence is required that a wide range of relevant, high quality and recent literature has been chosen and analysed in support of arguments. The marker will examine essays by assessing whether you have addressed the question in full, engaged in critical analysis of the issues, and supported arguments with the relevant and high quality literature. Please observe the assignment word limit, as word count that exceeds or is below the word limit will
attract a penalty. A 10% word count outside of the specified word limit is generally accepted however, you will
be penalised at the rate of 5% of available marks for every further 10% outside the word limit.
