You have recently secured a role as a Registered Nurse working with the World Health Organisation (WHO). Congratulations! This is your dream job. WHO are quick to put your skills and knowledge to good use and ask you to prepare a Community Profile Report.

Using the ten categories of the ‘SDH Assessment Circle’ as a guide (Clendon & Munns, 2019, p. 132), describe the health of a community of your choice.

You may choose 5 SDHs to discuss in your report. Based on your skills and knowledge, please conclude your report with basic recommendations.

ounding and environment, in which they dwell in and survive. They are generally mentioned as the social determinants of health (SDH) that compasses various conditions, facilities, resources as well as various opportunities that are accessible to the community (McMu
ay & Clendon, 2015).
The West Moreton Health (WMH) community profile report in this study helps in providing
ief and precise picture of the public health services, health status of the population, as well as priorities of the health care services provided to the people residing at West Moreton.
Geographical Position and Physical Determination of Health
The West Moreton region of Queensland, Australia, includes an entire rural part of the western area of South East of Queensland. The region rests in non-coastal region from the metropolitan area of Brisbane as well as the Gold Coast and towards the east of the Darling Downs of Australia (Queensland Government, 2020). Majority of the area of West Moreton falls in the Great divide range of Australia.
The health services for public in the West Moreton region are available to a number of inhabitants of more than 290,000 people approximately dwelling in the region su
ounding 9,521 square kilometres and expanding from Springfield, Ipswich as well as Ripley in the east and Boonah in the Southern region (Mactaggart, McDermott, Tynan & Gericke, 2018). The area also extends towards Esk in the northern part and towards Gatton in the southern region.
Biological Factors and the Population of West Moreton
The residential population of West Moreton was calculated to be around 286,271 people as of 30th June 2017. The data was observed to be portraying about approximately 6% of the population of Queensland (Ram, 2019). The area for West Moreton region had a larger segment of inhabitants, as compared to the population of Queensland of teenagers and young children, who are 0 years to 19 years of age. A smaller segment of old people from the age group of 50 years and above are there, as well as a section of young adults who fall in the age group of 30 to 34 years of age.
