In the provision of mental health care, the multidisciplinary team (MDT) may extend beyond health care professionals and may include peer support workers, carers, teachers, non-government supports and community health services.

For this assessment you are required to choose one (1) of the two (2) lived experience video scenarios, to be posted on , both of which demonstrate the potential for the development of problematic substance use, which may interact with the presenting mental health concern.

In addressing this topic, you are required to:

1. Identify three issues demonstrated in the video which may have contributed to problematic substance use. Ensure you support your choice of issues using current literature;

2. Discuss your chosen issues, using the principles of trauma-informed care and practice (TICP)(approx. 750 words);

3. Identify how your chosen issues should inform the mental health nurse’s collaboration with the person to achieve their personal recovery(approx. 750 words)and

4. Critically discuss how the nurse would apply trauma informed care principles, when engaging with the consumer. Choose another member of the mental health multidisciplinary team with whom the mental health nurse would engage in the process of collaborative care planning. Provide a rationale for your choice, using current literature.(approx. 300 words)

Introduction
Bipolar Disorder also called Manic Depression is related to episodes of extreme mood shifts which ranges from high manic phase to low depressive phase. The disorder is characterized by unusual variation in mood, energy and activity level.
The study evaluates the case study of former FootBall Player ‘Mark’, his mental health issues and its potential impact on which contribute to the problematic substance use. The player got diagnosed to have depression in addition to bipolar disorder when he was 43 however Mark had related symptoms since an early age.
According to World Footballer’s Association (2014), 26% of the football players suffer from mental health problems like distress, depression, low self esteem and therefore the adverse alcohol addiction.
The structure of the essay involves: (1) The issues which increase the risk of problematic substance use; (2) Discussion related to TICP principles; (3) Role of Mental Health Nurse; (4) The application of TICP Principles by Nurses in Mark’s case study.
Three Issues
1. Repeated Traumatic Condition: When the player had a bad preseason at the game of 1983, he experienced trauma. After a short high phase, he had a phase of depression in 1991 as he said he had a tendency to cry in the toilets prior to the game. These repeated sessions of high and low self esteem makes the patient hopeless, unconfident and undisciplined, also developing self stigma in behavior.
2. Absence of any awareness of Mental Issues: Though Mark had mental problems since an early age, he was not aware of it. This lack of awareness led to problematic substance use.
3. No Family Support and Isolated Life: Mark had been living away from the friends and family. In daily life, he could not share his feelings. His socially isolated and lonely life contributed to substance use.
Discussion
The Principles of Trauma Informed Care and Practice (TICP) involve transparency, safety, support and mutual help; collaboration and mutuality; historical and cultural issues; empowerment, voice and choice (Substance Abuse and Mental Health Services Administration [SAMHSA], 2014).
During the early age, Mark’s encountered trauma due to recu
ent lows in performance at sports. He thought it as flu like symptoms. He was incapable of training, walking, talking and performing properly. It was the initial phase of his mental issues that he could not identify. People used to ask him not to feel so
y about his condition. At the rehabilitation Centre, when he was 43, he was assessed with Depression and Bipolar Disorder. TICP emphasizes on developing transparency and trust with the patients by including the patient in the treatment. The patient requires being physically and psychologically safe. His needs need to be understood and strengths should be identified while his experience should be respected.
The Self Stigma is related to the perception of the patient about his own abilities (Mental Health Commission of New South Wales, 2019), Patient loses his interest in doing the things that they want to do, and feel lonely and isolated. The self stigma is cured by understanding the patient’s illness, recognizing his strengths and discussing the feeling of self stigma with the others who understand you. The care providers should identify the things that develop positive energy in the victim and improve his moral. The client can be allowed to do the things that he likes. The care providers can discuss the stories of other people who have been victims of the bipolar disorder so that the victim could take his own condition normally. Peer support and assistance from friends can help develop trust, and empowerment in the patient. The TICP principles can help to cultivate positive emotions in the patients.
Generally there is an integration of biological, social and mental factors which adds to the substance use. The client’s past experiences with trauma, economic wellness and social stigma contributes effectively to initiate the substance use. Around 50% of addicted people face mental and physical health issues, social stigma and social un-acceptance.
