Those of you who undertook NSG2104 in Semester 1 will remember you were required to conduct a literature review based on one of the National Safety and Quality Health Service Standards. Taking the Health Services Standard you selected, you are now required to develop a quality improvement project based on the Health Services Standard you addressed previously.

The content of the proposal will include: the background of the project; statement of the problem; significance or importance of the problem; a brief review of related literature; the premise of the project; the aim of your QI project, assumptions, definitions and measurements; outline of the project procedure and the use of appropriate improvement approach (PDSA for example); how findings might be disseminated. See the suggested framework (below) for how you might present your proposal.

Background of the Study
Bedside handover is a process in the nursing practice in which there is an interaction between nurse and the patient. The bedside nurses have the complete details about the patient. once nurse have the knowledge about the patient, when the shift change nurse handover the complete detail to the new bedside nurse who is going to take over former nurse place (Hada, Coyer & Jack, 2018).
Cu
ently, in this hospital facility, the rate of the bedside handover e
ors is 75% as per the annual report of the hospital management. In past one month, during the change in a shift, there are one or two cases of bedside handover e
ors by nurses. Apart from this, 60% of the patients failed to participate actively in the communication process.
One of the major issues identified is the failure of standard protocol which nurses must be followed while providing information about the patient to fellow nurse. As suggested by Manser and Foster (2011), use of effective communication strategy can reduce the e

Problem Statement
    As per the Australian Commission on Safety and Quality in Healthcare, it is necessary for practice bedside handover with active participation of the patient in nursing practice so that the new nurse who take over the duty have complete idea about the condition and need of patient, so the nurse can plan the type of care that should be given to the patient (Australian Commission on Safety and Quality in Healthcare, 2019).
Ideally, nurses who are going to handover the duty to other nurse must discuss thoroughly about the patient’s condition, which include communication with the nurse. As mentioned by Kassean and Jagoo (2005), there are frequent bedside handover e
