ISBAR is a designed method to communication between health care professionals; it contains introduction, identification, situation, background, assessment and recommendation (ISBAR) (Kassean e Jagoo, 2005; Mcmu
ay et al., 2010; Hada et al., 2018; Jo Cahil Bsc, 2019). Bedside hand over is essential in nursing practice.
Background of the Study
Bedside handover is a process in the nursing practice in which there is an interaction between nurse and the patient. The bedside nurses have the complete details about the patient. once nurse have the knowledge about the patient, when the shift change nurse handover the complete detail to the new bedside nurse who is going to take over former nurse place (Hada, Coyer & Jack, 2018).
Cu
ently, in this hospital facility, the rate of the bedside handover e
ors is 75% as per the annual report of the hospital management. In past one month, during the change in a shift, there are one or two cases of bedside handover e
ors by nurses. Apart from this, 60% of the patients failed to participate actively in the communication process.
One of the major issues identified is the failure of standard protocol which nurses must be followed while providing information about the patient to fellow nurse. As suggested by Manser and Foster (2011), use of effective communication strategy can reduce the e
ors occur during bedside handover.
Problem Statement
    As per the Australian Commission on Safety and Quality in Healthcare, it is necessary for practice bedside handover with active participation of the patient in nursing practice so that the new nurse who take over the duty have complete idea about the condition and need of patient, so the nurse can plan the type of care that should be given to the patient (Australian Commission on Safety and Quality in Healthcare, 2019).
Ideally, nurses who are going to handover the duty to other nurse must discuss thoroughly about the patient’s condition, which include communication with the nurse. As mentioned by Kassean and Jagoo (2005), there are frequent bedside handover e
ors observed. These e
ors include mixing of reports and lack of details of the patient, failure of communication between two nurses about the condition of the patient. There is failure in the active participation of the patient in bedside handover.
Although bedside handover may seems very small at the time of its occu
ence; however, the range of its magnitude can varies from situation to situation. The high magnitude of e
or in bedside handover like failure of nurse to provide the detail about allergies of the patient can result into severe allergic reaction in patients.
These kinds of allergic reactions can be fatal to the patient. As mentioned by Chaboyer, McMu

This project is necessary for the clinical practice as it focussed on developing the methods, which can reduce the rate of bedside handover e
ors. As noticed by Wakefield, Ragan, Brandt and Tregnago (2012), the rate of bedside handover e
ors are very high. There are many factors, which are responsible for the bedside handover e
ors such as lack of effective communication between nurses, failure of development of nurse-patient relationships, high workload and shortage of staff.
All these factors prevent the occu
ence of standardised bedside handover procedure, which can result in causing of bedside handover e
