The 1999 Institute of Medicine (IOM) report: To err is human: Building a safer health system was a wake up call for both the general public and healthcare providers regarding the problem and tragic consequences of medical errors. According to the Report Brief: "At least 44,000 people, and perhaps as many as 98,000 people, die in hospitals each year as a result of medical errors that could have been prevented" (Institute of Medicine, 1999, p. 1). The report recommends a four-tiered approach as a strategy for addressing this problem.

The Four IOM Recommendations
The first recommendation given was directed to Congress. By creating a Centre for Patient Safety, there will be a designated agency of the government devoted to monitoring and improving safety all through the system of health care delivery. This would form a national focus towards the creation of protocols to enhance knowledge about safety (Fraiser et al, 2012). The second recommendation advised that, under the mandatory reporting system, all States should be required to collect standardized information about events that lead to death and grave harm.
A third recommendation was aimed at the professional societies who should lead and encourage, if not demand standards of good practice amongst themselves oversight or safety net organizations are playing a major role in patient safety by implementing regulatory measures. Group purchasers of healthcare are in power to set financial incentives to healthcare organizations to make necessary changes. The fourth recommendation is that the healthcare workforce should work in collaboration with patients in order to create and fund adequately a system for monitoring patient safety.
